Clinical analysis of Cervical Spinal Fractures


Abstract
A detailed review of 47 consecutive patients placed in management and treatment for cervical spinal fractures by the Department of Neurosurgery, Pusan National University Hospital, during 3-year period from January 1, 1988, through December 31,
1990,
was carried out, and following results were obtained.
1. Fifty-one per cent of cervical spine fractures occurred in third and forth decade, and there was a male preponderance with sex ratio of 3.3 : 1.
2. The most common cause was motor vehicle accidents(44.7%), followed by falls(29.8%).
3. The most frequent mechamism of injury was flexion-rotation injury(46.8%).
4. The most frequent dislocation level was C4-5(25.5) and the most frequent level of body fracture was C5(12.8%).
5. Spinal fractures occurred most frequently in May(19.1%), followed closely by January. Twenty-three per cent of fractures occurred on Saturday, with 15 per cent and 15 per cent happening on Sunday and Tuesday, respectively.
6. At a initial examination, 39 patients(83%) had neurologic deficits. 19(40.4%) of the patients had a complete paralysis, 16(34.0%) of the patients had a incomplete paralysis and cases had root syndromes were 4(8.5%).
7. Of this group, 31(66%) had associated major injuries consisting of skull fractures and facial fractures, cerebral contusions and intracranial clots, thoracic injuries, major extermity fractures, intraabdominal injuries, thoracic and lumbar
spine
injuries.
8. Five patients(10.6%) were died because of pneumonia, sepsis, hypoxic brain damage and upper gastrointestinal bleeding.
9. There were no evidences of neurological recovery in completely paralyzed patients but some neurological recovery(75%) was found in both incompletely paralyzed cases and root syndrome cases. There was no significant difference between results
of
conservative and operative treatments.
